1. Click here: SPQ Gold Test then "Take Test."
2. When the SPQ* GOLD ONLINE screen appears, fill in the required text fields and optional fields as desired, then click the "Continue" button. Access Codes can be obtained from Julia Bremer by email or at 716-817-7109.
3. Read the questionnaire instructions and begin the questionnaire. At the end of each screen of questions, click on the "Next Page" button to continue. You may go back to review your previous page answers by clicking on the previous page button on the screen. DO NOT USE YOUR BACK BUTTON. NOTE: Blanks are not permitted either.
4. When you reach the end of the questionnaire, click on the "Submit" button. Your test will not be scored until the "Submit" button is clicked.
5. A "Thank You" screen will appear to let you know your test has been received for processing.
